Incentive Payment Process for Veterinary Clinical Trials

The Ohio State University Veterinary Medical Center (OSU-VMC) uses a Payee Setup Process to manage payee information in its payment system. Incentive payments for participating in a clinical trial at the OSU-VMC can only be issued to payees listed in this system. To meet Internal Revenue Service and State of Ohio requirements,  specific information must be collected from all individuals receiving payments.

Those who have not yet completed the Payee Setup Process with the OSU-VMC will need to complete a Payee Setup Form and submit it by email to CVM-vetclinicaltrials@osu.edu, and type "osusecure" in the subject line to send it as an encrypted message. Please ensure all documents are fully completed and attached to avoid delays. Acceptable formats of the completed Payee Setup Form include a scanned hard copy, PDF, or JPG file. This step is required before any payments can be processed. 

Payment Processing:

Incentives will be submitted for payment 3-5 business days after receiving confirmation that the pet has completed the clinical trial's required endpoint. Payment can take up to 3 weeks to arrive after it has been submitted.

  • Pages 1 and 2 are required to be completed
  • Select Clinical Study Subject from the misc. reasons box on page 2 and Net 90 payment.
  • Be sure to sign and date all completed pages.
  • No voided check is necessary. 
  • Pages 1, 2, 3, and 4 are required to be completed.
  • Select Clinical Study Subject from the misc. reasons box on page 2 and Net 60 payment.
  • Be sure to sign and date all completed pages.
  • Provide a copy of a blank Voided check that matches the banking information entered on the form. 
  • You do not need to complete the form.
  • Payment will be processed through HR and distributed in the next payroll cycle.
  • Note: taxes will be calculated and withheld as necessary.
